It’s important that we have the correct contact details for you.
This is because they are used by your GP and other NHS services to contact you about screening, vaccinations or appointments.
You can update your contact details:
- online using the NHS App – this is for for email and mobile phone changes only
- online using Engage Consult & selecting ‘submit an admin query’
- by calling our reception team
- by coming into the surgery and completing a patient update form
We might need to ask you for your ID or proof of the change e.g. if you change your name through marriage or Deed Poll, the Department of Health require us to see your documentation. We will advise you what is required at the time.
Moving out of our area
If you move outside the practice boundary you will need to register with a new GP surgery. Our practice boundary is available here: Practice boundary
