Social Prescribers

There are times when we all need support to address life’s challenges. Knowing how to access available support locally can make a big difference.

Social Prescribing helps you to explore services, groups and activities that can support you to improve your health and emotional wellbeing, for example:

  • healthy lifestyles
  • arts, music & creativity
  • befriending, counselling & support groups
  • Carers support
  • housing, benefits and financial support & advice
  • employment, training & volunteering
  • education & learning
  • getting involved with local groups
  • accessing specialist groups or services

We have a Social Prescriber who works for us and the other GP surgeries in Rural Rother as part of our Primary Care Network. She has appointments at Battle Health Centre on a Friday morning. To find out more and self-refer, visit the Rural Rother PCN website.

Community Connectors

Social prescribing in East Sussex is provided by Community Connectors. Their Team of Community Navigators will work with you to provide support and encouragement and create a personalised plan to help you connect with services in your community.
